MFB Youth Summer Camp Sign Up: 2-7 June 2024
Looking forward to summer camp 2024 at Look Up Lodge for those finishing 6th through 12th grade!

We'll leave the church on June 2nd after the service like last year. Pick up is 10:00-10:30am at camp or around 11:00-11:30 at the church on the 7th. 

$400 Early Bird before 1 Jan 2024
$425 before 1 Mar 2024
Final Sign Ups due by 1 March to finalize payments and spots with Look Up Lodge and organize enough leaders to join us.

(I realize the cost has gone up quite a bit. Talking to the directors at Look Up Lodge, it's mostly due to the significant cost of food. They are doing their best to still keep the cost low overall as much as possible. We can work out a payment plan to help budget for this as well as do some fundraising events.)

To confirm your spot, a $100 non refundable deposit is required after signing up and full payment due by May 1st. Please give checks or cash to Rebecca or Tory and see them if you'd like to pay by card.

LIABILITY RELEASE: I, the Legal Guardian give consent for the participating student to attend any or all of the events held by Mauldin First Baptist. I/We understand that there are inherent risks involved in any ministry or athletic event. I/We release Mauldin First Baptist, the pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of these activities. In the event that he/she is injured and requires medical attention, I/we authorize Mauldin First Baptist to obtain medical treatment for the student and the Emergency Contact on this form will be called. I/We have read and understand this agreement and have willingly placed my name as an electronic signature below as evidence of my acceptance of all the conditions contained herein. *
PHOTO RELEASE: By participating in any of the activities, my child will probably be photographed and/or videotaped during normal activities by Mauldin First Baptist or Look Up Lodge Camps. Those photographs or videos may be used on social media or promotional materials for Mauldin First Baptist or Look Up Lodge Camps.
