MFB Kids - A Legendary Nite: Episode 8
This is wild and crazy evening for K5-5th graders. Join us for a $2 pizza dinner at 5:30, followed by some messy games, worship, and bible teaching! You'll definitely need extra clothes and a towel! Make sure to bring your friends to join in the fun.

Due to increased attendance and the need for preparation time,  we will be closing registration on Tuesday, April 11th  Register Now!

LIABILITY RELEASE: I, the Legal Guardian give consent for the participating student to attend any or all of the events held by Mauldin First Baptist. I/We understand that there are inherent risks involved in any ministry or athletic event. I/We release Mauldin First Baptist, the pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of these activities. In the event that he/she is injured and requires medical attention, I/we authorize Mauldin First Baptist to obtain medical treatment for the student and the Emergency Contact on this form will be called. I/We have read and understand this agreement and have willingly placed my name as an electronic signature below as evidence of my acceptance of all the conditions contained herein. *
PHOTO RELEASE: By participating in any of the activities, my child may be photographed and/or videoed during normal activities and that the photographs or video may be used in promotional materials and on social media for Mauldin First Baptist. *
CONCERNING COVID-19: Mauldin First Baptist cannot prevent you or your child(ren) from becoming exposed to, contracting, or spreading COVID-19 while utilizing the services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize Mauldin First Baptist's services and/or enter onto Mauldin First Baptist's premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19. ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19.
